Our patients are assigned a “whole patient” care team and have seven day a week access to app-based care, using Remote Patient Monitoring (“RPM”) to bill under the patient’s insurance. What You Will Do Deliver exceptional care centered around the core pillars of lifestyle change management (nutrition, movement, stress management, sleep, behavioral change) and rooted in evidence-based counseling Work closely with a multidisciplinary team, including fellow registered dietitians, licensed nurses, LCSWs, medical assistants and gastroenterologists Support patients with both synchronous and asynchronous care, utilizing the core tenets of motivational interviewing to enhance behavioral change Adhere to clinical protocols and evidence-based practice guidelines Requirements RD/RDN, Multiple state licensure a plus 2+ years experience as a an RD/RDN Working knowledge of nutritional approaches for GI conditions (IBS, IBD, SIBO) a plus Bilingual (spanish speaking) a plus Experience communicating with patients asynchronously preferred Passion for working with clients to facilitate behavior change by helping them set health goals to effectively manage chronic disease.
