It's the handful of well-designed hours that help someone living with a chronic illness or a mental health condition get back to the life they want, and the clinical judgement to recognise when they need something more. Our ANZ Connected Care Programs deliver insurer-funded mental health and chronic disease support to people right across Australia. We're growing, and we're bringing health coaching in house. Deliver structured, time-limited health coaching by video and telephone across our mental health and chronic disease programs Complete baseline and completion clinical assessments, screen for risk, and escalate confidently when a participant needs more Work with people managing depression, anxiety, cardiac and metabolic conditions, persistent pain, and the psychological weight of long-term illness Write end-of-program reports for referrers and treating teams that are clear and useful Contribute to a genuine multidisciplinary team and to how these programs are designed as they scale You hold general registration as a psychologist with AHPRA, with a Master's or Doctorate in clinical, counselling or health psychology You have at least five years post-full-registration clinical experience, including coaching You have genuinely practised health coaching under a recognised coaching model — not therapy with a coaching label You understand how physical illness and mental health shape each other You're excellent remotely, organised on paper, and generous in a team You are living in Australia with the right to work here An ICF credential or equivalent coaching psychology qualification Experience supporting people whose physical health concerns affect their wellbeing, including chronic disease Five or more years in psycho-oncology, which opens up work in our Cancer Support stream A strong multidisciplinary model.
