 Description: 
 Position Summary
 The Registered Dietitian serves as a key member of the Trinity Health HEART Center team, directing the nutritional components of the HEART Center, Medical Weight Management Program, and Diabetes Education Program. This role is responsible for planning, organizing, evaluating, and coordinating nutrition-focused programs that promote healthy lifestyle behaviors, weight management, cardiovascular health, and diabetes self-management.
 The ideal candidate is a passionate nutrition professional who combines clinical expertise with program leadership, patient education, and collaborative care to improve health outcomes across diverse patient populations.
 Key Responsibilities
 Clinical Nutrition Management
 Conduct comprehensive dietary assessments and develop individualized nutrition care plans.
 Implement nutrition treatment interventions and evaluate patient progress toward established goals.
 Collaborate with physicians, managers, and interdisciplinary care team members to ensure exceptional patient care and outcomes.
 Provide nutrition counseling and education for patients with cardiovascular disease, diabetes, obesity, pulmonary conditions, eating disorders, and other nutrition-related diagnoses.
 Patient Education & Counseling
 Develop, implement, and evaluate educational materials and programs related to nutrition, weight management, diabetes prevention and management, and healthy lifestyle practices.
 Facilitate individual counseling sessions, support groups, nutrition classes, and patient group meetings.
 Educate patients and families on nutrition principles, disease management, behavior modification strategies, and self-management techniques.
 Serve as a nutrition resource for patients, providers, and community stakeholders.
 Program Development & Leadership
 Coordinate and oversee nutrition-related programming within the HEART Center, Medical Weight Management Program, and Diabetes Education Program.
 Develop nutrition care processes and protocols for targeted patient populations.
 Participate in care conferences, discharge planning, and multidisciplinary treatment planning.
 Act as a nutrition liaison for the Trinity Health primary care network, supporting nutrition education programs and workshops for colleagues and patients.
 Quality Improvement & Compliance
 Participate in Continuous Quality Improvement (CQI) initiatives and quality assurance activities.
 Maintain compliance with organizational policies, accreditation standards, and regulatory requirements.
 Accurately document patient assessments, treatment plans, progress notes, educational activities, and outcomes in the electronic medical record.
 Support resource utilization and evidence-based practice initiatives to improve patient outcomes.
 Professional Leadership
 Serve as a role model for healthy living and wellness.
 Foster collaborative relationships with healthcare team members and community partners.
 Participate in committees, task forces, research activities, and community outreach events.
 Pursue ongoing professional development and continuing education to maintain clinical expertise.
 Required Qualifications
 Bachelor's degree in Dietetics, Nutrition, or related field.
 Registered Dietitian (RD/RDN) credential.
 Current membership in the Academy of Nutrition and Dietetics.
 Certified Diabetes Care and Education Specialist (CDCES) or Certified Diabetes Educator (CDE).
 Current Basic Cardiac Life Support (BCLS) certification.
 Knowledge of Joint Commission standards, quality assurance programs, and compliance requirements.
 Experience in: Cardiovascular disease prevention and management
 Clinical nutrition therapy
 Adult and adolescent weight management
 Diabetes education
 Health education and behavior change strategies
 Coronary artery disease risk factor management
 
 Experience leading programs, facilitating groups, and coordinating patient education initiatives.
 Preferred Skills
